A Clincal Analysis of Carcinoma of the Gallbladder



Abstract
Since carcinoma of the gallbladder often presents bizarre and obscue clinical manifestations, the correct preoperative diagnosis is difficult to make. Despite recent advance in medical knowledge and surgical techniques, little progress has been
made in
early diagnosis or surgical treatment of carcinoma of the gall bladder. The atuhor experienced 55 cases of gallbladder carcinoma proven histologically and treated surgically in department of surgery, Pusan national University Hospital from
January,
1981
to December, 1990.
@ES The results are as follows;
@EN 1) Male to female ratio was 1 : 1.2 and it was seen most common in the age aroup of 50s and 60s(mean age ; 58.5).
2) The clinical symptoms were abdominal pain, weight loss, anorexia, jaundice, palpable abdominal mass.
3) The duration of symptoms was less than 6 months in 38 cases(69.1%).
4) The laboratory findings were as follows, SGOT was elevated in 18 cases, SGPT in 20 cases, alkaline phosphatase in 12 cases, total bilirubin in 15 cases.
5) In diagnostic methods, ERCP, ultrasound, abdominal CT scan were performed, and preoperative diagnostic accuracy was 43.6%.
6) Operative procedures were cholecystectomy(27.3%), cholecystectomy with incontinuity resection of hepatic bed(21.8%), biopsy(20.0%), cholecystectomy with T-tube choledochostomy(18.2%), etc.
7) The most common metastatic site was lever(36.4%), followed by regional lymph node, greaster omentum, CBD etc.
8) The histologic findings were adenocarcinoma(92.7%), squamous carcinoma(7.3%). The prognosis was very poor, 83.6% died within a year of diagnosis.
